MEDIA CREDENTIAL REQUEST FORM

All media wishing to attend the press conference must complete the following form in its entirety.  Due to the number of expected requests coupled with space limitations, all requests may not be fulfilled.  Please note that priority will be given to those working on deadline.  Please fax completed form to the attention of Danielle Moorman at (817) 335-7737 no later than Mon., Feb. 12.  Approved parties will receive e-mail notification on or before Fri., Feb. 16.  Credentials will be available for pick-up beginning at 3:30 p.m. on Mon., Feb. 19.  Credentials are non-transferable.

As a rule, admittance to the press conference will be allotted on a first come, first serve basis to:

  1. Local/regional television reporters and photographers
  2. Local/regional daily/weekly newspaper sports writers, columnists and photographers
  3. Photographers and columnists from lifestyle magazines
  4. Regionally-based beat writers/stringers for national wire services and media outlets
  5. Writers from established, nationally recognized Web sites
  6. Approved media relations representatives associated with Ohio State

* Freelance and amateur photographers are not eligible for media credentials.
* Credentials provide access to the press conference only.  Tickets to the cocktail reception
and awards dinner must be purchased.
